As part of your business relationship with Heritage Baptist Church you are required to provide insurance certificates evidencing coverage as follows:

HBC All Other Subcontractors

Please submit a certificate of insurance that meets the following requirements:
* General Liability insurance with minimum limits of $1,000,000 per Occurrence, $1,000,000 Prod/CO Aggregate, and $1,000,000 General Aggregate. If your policy limits are less than these requirements, your certificate should include any excess or umbrella policies that extend the general liability limits.
* General Liability must cover Premises and Operations, Products and Completed Operations, Broad Form Property Damage, and Personal/Advertising Injury.
* The Certificate holder MUST be named as an additional insured on the General Liability coverage's with an acceptable endorsement attached and the endorsement form number indicated on the certificate.
*Additional Insured Endorsement(s) must cover both ongoing and completed operations, and are subject to approval by Taylor Morrison.
* General Liability must include a waiver of subrogation and indicate on the certificate.
* Auto Liability insurance with minimum limits of $500,000 Combined Single Limit, covering All Owned, Hired, and Non-Owned vehicles. If your policy indicates Scheduled Autos, your certificate must be accompanied by a letter from your firm's principal owner or CEO, stating that the ONLY vehicles allowed on the Taylor Morrison job site will be the scheduled vehicles.
*Auto Liability must name certificate holder as additional insured.
* Workers Compensation providing statutory coverage limits. Workers Compensation must also include a waiver of subrogation noted on the certificate.
* Employers Liability insurance with minimum limits of $1,000,000 Occupational Disease, $1,000,000 per Accident, $1,000,000 per Employee, also including a waiver of subrogation noted on the certificate.

Heritage Baptist Other Subcontractors Can't

* All certificates must include an authorized signature and provide for at least 30 days notice of cancellation.
* All of the above coverage's must be written by a carrier with a minimum A.M. Best rating of A- or better
* Any deductibles or self-insured retentions should be noted on the certificate.
* Claims-made coverage's are not acceptable
* Blanket naming waiver of subrogation is acceptable.

Heritage Baptist Church Low Risk Trades

Please submit a certificate of insurance that meets the following requirements:
* General Liability insurance with minimum limits of $500,000 per Occurrence, $500,000 Prod/CO Aggregate, and $500,000 General Aggregate. If your policy limits are less than these requirements, your certificate should include any excess or umbrella policies that extend the general liability limits.
* General Liability must cover Premises and Operations, Products and Completed Operations, Broad Form Property Damage, and Personal/Advertising Injury.
* The Certificate holder MUST be named as an additional insured on the General Liability coverage's with an acceptable endorsement attached and the endorsement form number indicated on the certificate.

* All of the above coverage's must be written by a carrier with a minimum A.M. Best rating of A- or better
* Any deductibles or self-insured retentions should be noted on the certificate.
* Claims-made coverage's are not acceptable

Heritage Baptist Church Professional

Please submits a certificate of insurance that meets the following requirements:
* General Liability insurance with minimum limits of $1,000,000 per Occurrence, $1,000,000 Prod/CO Aggregate, and $1,000,000 General Aggregate. If your policy limits are less than these requirements, your certificate should include any excess or umbrella policies that extend the general liability limits.
* General Liability must cover Premises and Operations, Broad Form Property Damage, and Personal/Advertising Injury.
* General Liability must include a waiver of subrogation and indicate on the certificate.
*Professional Liability with minimum limits of $1,000,000 each occurence, and an aggregate limit of $1,000,000.
*Professional Liability must be provided under a claims-made coverage form with retroactive date indicated.
* All certificates must include an authorized signature and provide for at least 30 days notice of cancellation.
* All of the above coverage's must be written by a carrier with a minimum A.M. Best rating of A- or better
* Any deductibles or self-insured retentions should be noted on the certificate.
* Blanket naming waiver of subrogation is acceptable.
